| Dear Rob, Making your own fax coversheet is great! You get the chance to customize the first bit of information a person gets to see about yourself, or your company. We suggest you put it in any format that you like that is easy to read, but include the following items on your fax coversheet: From Name From Business/Department From Fax Number Date To Name To Business/Department To Fax Number Subject Sheet Count, including the cover sheet Extra information section. The extra information section is kind of an ad hoc section for your fax sheet. This is where you would include instructions like, please shred after use, or if you want a reply via email, phone, or another fax number.
